Main duties of the job
The Complex Discharge team at the University Hospital Southampton NHS Foundation Trust (UHS) are looking to appoint to an opening for a Band 7 Team leader role, initially on a fixed term/ secondment opportunity. The role is fast-paced, varied, and incredibly interesting, supporting the wider hospital and enabling patients to progress closer to their discharge following acute care. We are looking for someone with good communication skills, who is reliable and driven, with a clear passion for excellent patient care. The post holder will have a variety of leadership responsibilities within the immediate team and also hold a diverse patient caseload according to the service needs. We are a really friendly team and like to support one another and have fun in our work, helping patients to remain independent and have a safe, quality discharge on leaving our hospital.

Detailed Job Description And Main Responsibilities
The post holder is responsible for managing the teams undertaking the assessment and discharge planning of patients within the defined clinical area, working under the direction of their Complex Discharge Operational Lead. In addition, act as a case manager using their clinical knowledge and experience to support the day-to-day operational functions of the teams, completing their own assessments and interventions where indicated.

Essential criteria
- Professional Registration in an appropriate health care discipline.
- Degree (health related) or equivalent qualification/training and experience.
- Recognised teaching/mentorship qualification or equivalent experience
Essential criteria
- Professional and clinical knowledge around complex discharge issues, or a willingness to develop such knowledge.
- Broad range of post registration experience within an acute hospital setting.
- Experience in involvement with complex discharge.
- Basic knowledge of the roles and responsibilities of Social Services
- Working knowledge of the legislation concerning complex discharge
- Understanding of personal accountability
- Experience of supervising junior staff members
- Demonstrates clarity and breadth in use of written communications in relation to our patient group
- Ability to participate in research in relation to role in complex discharge
- Ability to liaise with internal and external stakeholders to co-ordinate complex discharge plans
- Experience in using the APEX complex discharge database
- Experience of teaching
- Knowledge of current issues within Healthcare and Social Services
- Knowledge of how health care, social care, voluntary ,private and clinical commissioning groups work together
- Experience of contributing to the investigation of complaints
- Experience of leading planning meetings
