This post is fixed term/secondment for 6 months due to the needs of the service.

As a Band 7 Discharge Hub/Transfer Lounge Coordinator, you will play a pivotal role in ensuring the safe and efficient discharge of patients from our hospital. You will work closely with multidisciplinary teams to streamline discharge processes, reduce delays, and enhance patient experience.
Main duties of the job
Lead and manage the Discharge Hub/ Transfer Lounge team, providing guidance and support.
Coordinate with healthcare professionals to develop and implement discharge plans.
Monitor and evaluate discharge processes to identify areas for improvement.
Liaise with external agencies to ensure seamless transitions of care.
Maintain accurate records and ensure compliance with relevant policies and procedures.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
