We are looking to appoint an enthusiastic, motivated and capable person to the above position. The successful candidate will have good communication skills and work as part of a team in the delivery of a rehabilitation service to promote recovery for people with a recurrent and enduring severe mental illness. There must be a commitment to develop knowledge and skills.
This eight-bedded unit situated in Wrexham is recognised as a vital and integral component of the adult mental health service provision for Betsi Cadwaladr University Health Board.

This eight-bedded unit situated in Wrexham is recognised as a vital and integral component of the adult mental health service provision for Betsi Cadwaladr University Health Board.
Main duties of the job
- To be solely responsible for the planning, implementation and evaluation of care needs.
- To undertake a range of assessments with the client to establish a baseline point.
- To prepare care plans to ensure the delivery of effective care.
- To prepare programmes of rehabilitation, ensuring the results of the assessments are recognised.
- To evaluate and update Risk Assessments as necessary.
- To compile full and comprehensive reports for client reviews, reporting on current mental state and progress within the rehabilitation programme.
- To liaise with Community Mental Health Team colleagues with regard to discharge planning.
- To communicate complex information to clients, relatives and other professionals in regard to key worked clients.
- To maintain client case notes to ensure information is accurate and up to date. To maintain section in case notes in regard to the Mental Health Act (if appropriate) to comply with regulations set down by the Mental Health Commission.
- To direct associate nurse in any of the above aspects if unable to so yourself because of shift restrictions (e.g. night duty).

Essential criteria
- 1st Level Registered Nurse RMN
- Professional/clinical knowledge acquired through training to degree/diploma level
- Have a wide knowledge of potential mental health, learning disability problems that occur within inpatients and/or community settings.
- Have a good working knowledge of the mental health, learning disability needs of service users and their carers
- Evidence of post registration study and training, e.g. Safeguarding; Mentor; Intermediate Life Support
Essential criteria
- Experience working as a mental health nurse and assessing service users health and well being and the supervision of other staff and systems
- A good understanding of the Mental Health Act/ Mental Health Capacity Act
- Possess an understanding and have experience of multi-agency and multi disciplinary working
- Clear understanding of clinical governance framework; Implement evidence based practice
Essential criteria
- To prioritize and organise working day
- Demonstrate the ability to supervise junior members of the team and to be able to demonstrate own safe practice in accordance with NMC regulations and Health Board policies and procedures
- Able to exhibit good de-escalation skills and feel confident enough to use these on a regular basis
