Solihull Metropolitan Borough Council are looking to recruit an experienced Social Worker to join their Hospital - Discharge to Access Team (D2A)

Your new role

This is an exciting opportunity for a qualified Social Worker who has proven experience in the hospital Discharge to Access Team (D2A). As part of an experienced D2A social work team, you will play a key role in shaping and delivering this service, providing a specialist social work service to Solihull residents who require support to leave a hospital setting or require long-term planning after a hospital admission.

As a qualified Social Worker, you will hold substantial post-qualifying experience in Adult Social Work Services below.

  • Knowledge on completing long term assessments and discharges for individuals who are on discharge to assess pathways.
  • Experience of safeguarding, complex case management and meeting and working within pressurised timescales.
  • Experience of managing positive risk-taking and making decisions.
  • Experience of multi-agency working.
  • Detailed knowledge of relevant legislation including: Care Act 2014, Mental Capacity Act 2005 (inc. DoLS), MHA 1983, etc.
  • Ability to effectively communicate complex and sensitive information both verbally and in writing, including effective report writing
  • Effective interpersonal skills and ability to respond positively to challenging situations

The role is to start asap, working 37 hrs per week (Monday – Friday) up until the end of Dec 2024 with the potential of permanent opportunities for the right candidate.

Driving is essential for the role as you will be carrying out assessments throughout the borough and neighbouring councils.

What You'll Need To Succeed

To be successful you will require, a Social Work qualification (BA Social Work, CQSW, DipSW, CSS or CCETSW approved equivalent).

  • Current professional registration with Social Work England
  • minimum of 1 year experience working in adult social work.
