What You Will Enjoy Doing
- You will be responsible for delivering tailored support to individuals attending Pulmonary Rehabilitation sessions in community clinics, working within clinical protocols and guidance
- As well as assisting in the setup, delivery and evaluation of rehabilitation programmes, you will help ensure sessions are welcoming, accessible and person-centred
- You will monitor patient progress, adapt support as needed and communicate any concerns or developments to the clinical team
- You will manage a caseload with care and efficiency, prioritising needs to provide timely and inclusive support
- As part of discharge planning, you will help patients build confidence and develop long-term self-management strategies
- You will contribute to a collaborative and respectful environment, supporting coordinated care across the multidisciplinary team
- Strong foundation in literacy and numeracy demonstrated through formal education
- NVQ Level 3 in Health and Social Care, Diagnostic and Therapeutic Support or an equivalent relevant qualification
- Evidence of recent professional or personal development in a clinical or healthcare setting
- Full UK driving licence to support community-based patient care
- Level 4 Certificate in Exercise Training, ideally focused on Chronic Respiratory Disease
- Degree or Diploma in Exercise Physiology with clinical application
- NVQ Level 4 or Foundation Degree in a clinically relevant subject
