The LDA Collaborative has been successful in receiving National funding to lead an exciting project to pilot a combined health check for people with a LD, autistic people and people with a SMI. As part of this project, we are looking for 2 dynamic clinicians who have experience in mental health, learning disability and/or autism. This post is a 1 year fixed term, secondments will also be considered.

Job Description Summary (Main Duties And Responsibilities)
The Primary Care Liaison Nurses (PCLN) will support in co-designing, developing & testing a combined health check template and associated resources. The clinicians will work with Primary care to identify approx. 500 patients to receive the health check; providing training and support to enhance the quality of the health check.
You will be part of formally evaluating the project and reporting the findings locally to the LDA Collaborative and Nationally to NHSE.
You will be based at County Hall alongside the existing PCLN team but will be out closely working with the identified Primary Care Services and GP’s across Leicester, Leicestershire or Rutland. Successful candidates will be working a 37.5 hours a week, Monday to Friday between 9-5; however some flexibility is required to work evenings and weekends to support training and events.
