We have an exciting opportunity for you to join our Connect Hospital to Home Response Team. You would become part of an ambitious team delivering an invaluable service to the residents of Middlesbrough. Connect is a Middlesbrough Council service which delivers the Community alarm services and Telecare services to people across Middlesbrough.
We offer help and support to older, vulnerable, and disabled people to help them to remain safe and independent in their own home. You will deliver support to residents who are being discharged from hospital. You will install essential equipment and provide services required to enable a safe and speedy discharge and to help maintain independence within their home. You will provide a high standard of person-centred care when responding to emergency situations. You will be trained in emergency first aid, moving and handling and the use of all lifting equipment. You will assess the service user's needs, offer support, and then decide on the most appropriate action to be taken. In this role, good communication skills are vital, also responding with empathy is important as well as providing the reassurance the service user needs.
A full UK driving licence is required for this post.
This vacancy is part time working 36.75 hours per week.
