Position:
We are seeking a passionate, proactive and compassionate Complex Needs & Housing First Case Coordinator to join our friendly team in Exeter. This role involves providing intensive, one-to-one support to individuals with complex needs — many of whom have experienced rough sleeping or long-term housing instability.
This is a role where you'llwork directly with customers in their homes and the community, building trusted relationships and delivering trauma-informed, person-centred support. You'llhave the autonomy to manage your caseload, backed by a collaborative and highly supportive team.

Key Responsibilities
- Individual Support: Build strong, trusting relationships with clients who may be facing complex barriers to stable housing.
- Housing First Approach: Deliver support rooted in the Housing First model — prioritising permanent housing and wraparound care without preconditions.
- Case Management: Develop personalised support plans based on each individual's goals, strengths and needs.
- Outreach & Advocacy: Provide support in a variety of settings, advocating for clients to access health services, mental health and substance use support, benefits, employment and education opportunities.
- Crisis Support: Offer calm, practical interventions during times of crisis, ensuring safety and wellbeing.
- Multi-Agency Working: Collaborate closely with local partners to ensure coordinated, holistic care.
- Safeguarding & Record-Keeping:Identify and respond to safeguarding concerns and maintainaccurate, timely case notes and documentation.

- Understand or are willing to learn about trauma-informed care and the Housing First model

Requirements:
- Experience: Experience working with individuals who have experienced homelessness or related services. Lived experience with housing instability or homelessness is highly valued and considered equivalent to professional experience.
- Skills: Strong interpersonal skills with the ability to build trust and rapport with diverse populations. Excellent communication, organisational, and problem-solving abilities.
- Knowledge: Familiarity with the Housing First model and trauma-informed care practices.
- Empathy & Compassion: A genuine desire to help people overcome barriers to stable housing and improve their quality of life.
- Flexibility: Ability to work independently and as part of a team, manage multiple priorities, and adapt to the evolving needs of clients.
