Job Overview
This is a part time (0.8wte) ACP role working across the frailty pathway. You will be responsible for delivering excellent care to patients throughout the specialty including the G.P. take within normal working hours with a view to supporting the development of an acute G.P accepting area with the admissions unit. The role will require the post holder to support the 32a nursing team with appropriate triage and initial review of G.P admissions, clerking, CGA and post take with a Geriatrician, available from 8am – 8pm. You will be required to liaise with community providers to access alternatives to admission such as frailty@home. Additionally you will be supporting the complex needs of patients in our wards which includes patients who require ongoing support in the community. You will be working as an independent practitioner with the support of an experienced team of consultants.
With a background of nursing, paramedic or physiotherapy, the post holder will have completed training to practice as an autonomous Advanced Clinical Practitioner. This will have incorporated the progressive development of history taking, clinical examination, clinical investigations and interpretation and subsequently the diagnosis and management planning of the patient specific to the clinical specialty of frailty and Medicine for Older Persons. The successful applicant will have completed an MSc in Advanced Clinical Practice or equivalent as stipulated by the RCEM ACP Curriculum.

Per annum, the trust treats over 100,000 inpatients, 35,000 outpatients, 100,000 emergency department patients and help deliver over 6,000 babies. It employs more than 9,500 staff and has approximately 1,050 inpatient beds.
Furthermore, the Trust has University Teaching status and is associated with both the University of Bristol and the University of the West of England. This allows all staff members to actively engage in the teaching of future generations of doctors, nurses, paramedics and many more.

- Whilst working within the professional boundaries of an ACP, assess, examine, investigate, diagnose and treat patients resulting in the safe management, which may consist of further management, referral to another specialty or discharge.
- Contribute and assist in safe and efficient patient flow and become involved in potential areas of improvement within this area.
- Be directly involved in patient experience quality assessment and implementation of improvement strategies.
- Critically assess patient care and plans and address any issues, which may result in substandard care.
- Be familiar and implement trust and national policy, guidelines and recommendations to facilitate a standardised approach.
- Ensure accurate and legible documentation and records are maintained in accordance with the Data Protection Act 1998.
- Practice in accordance with specific professional body codes of professional behaviour.
- Maintain infection control standards.

- MSc in Advanced Clinical Practice
- Active Professional Registered Qualification with a Professional body e.g., NMC, HCPC
- Non-medical prescriber
- Expert clinical experience at a senior level in Frailty and Care of the Elderly
- Expert clinical experience at a senior level in Frailty and Care of the Elderly
- Evidence of ongoing professional development through maintenance of professional portfolio and registration revalidation.
- Proven leadership experience
