We're looking for a Transitions Link Officer to become the vital link between schools, families and our College, ensuring that learners with Education, Health and Care Plans (EHCPs) and additional learning support needs experience a smooth, well-planned transition into further education.

About the Role

As our Transitions Link Officer, you'll be the first point of contact for schools, SENCOs, specialist settings and external professionals, working closely with our EHCP and Learning Support teams. You'll play a key role in gathering and sharing high-quality information that enables the College to understand learners' needs, plan effective support and make informed decisions through our consult and panel processes.

You'll also be a welcoming and knowledgeable ambassador for the College, helping families and professionals understand the opportunities available and the support we provide to help learners thrive.

What You'll Be Doing

  • Building strong relationships with schools, SENCOs, specialist settings, families and external agencies.
  • Gathering, recording and sharing accurate information about prospective learners with EHCPs or additional learning support needs.
  • Supporting the College's transition planning by ensuring Learning Support teams have the information they need to prepare for learner success.
  • Contributing to consult responses and supporting decision-making through the College's agreed panel process.
  • Providing advice and guidance to families and professionals about transition pathways, learning support and the College's processes.
  • Acting as a positive ambassador for the College, promoting an inclusive and learner-centred approach.

We're Looking For Someone Who...

  • Has experience working with young people with SEND, EHCPs or additional learning support needs.
  • Understands the transition process from school into further education.
  • Builds positive relationships quickly and communicates confidently with a wide range of people.
  • Is highly organised, with excellent attention to detail and the ability to manage sensitive information accurately.
  • Works collaboratively and enjoys being part of a supportive, multi-disciplinary team.
  • Is passionate about inclusion and committed to helping every learner achieve their ambitions.
