This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust and have not been an employee of the Trust in the previous 3 years. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays



An exciting opportunity has arisen for a dedicated Young Person Transition Practitioner Band 6 to join our growing team of multidisciplinary clinicians providing care to patients within Nottingham and Nottinghamshire. This is a specialist role working across both the Adult Mental Health and CAMHS Pathways.



This post will have a specialist role in facilitating transitions for individuals who have complex and enduring mental health problems from CAMHS to the Adult Mental Health Directorate. The post holder will develop innovative new ways of working with other providers to enhance improved working practices and relationships across healthcare in order to meet strategic objectives, organisational requirements and evidence-based practice developments



The post holder will hold a caseload of complex young people who require transition into adult mental health services; they will also liaise with agencies, creating positive and helpful relationships which promote a robust pathway.

To hold a complex position of straddling two services/teams and ensuring your practice meets the changing need of the individual and their carer.



Support the Young person practitioner lead, in development and embedding of an evidence-based pathway for transitions



To enhance access to the mental health services for young people for whom it is clinically indicated so to allow timely assessment, formulation and evidence-based intervention within a multi-agency framework



Work in partnership with CAMHS clinicians, other senior clinicians and wider networks, to promote a robust Pathway.



Co-ordinate and facilitate an appropriate discharge.



To take a lead role in transitioning young people to Adult Mental Health Services service and where appropriate provide therapeutic intervention.

Be proactive in supporting the leadership team influencing service development



Promote clinical excellence in line with local and national guidance whilst working within the parameters of Trust policy & procedure.



Support Young person transition lead in the development and embedding of an evidence-based pathway for transitions



Enhance and support other service professionals to promote and support the emotional health and wellbeing of children, young people and families through contributing to professional consultation and training specific to the needs of young people requiring mental health interventions



To enhance access to the mental health services for young people for whom it is clinically indicated so to allow timely assessment, formulation and evidence-based intervention within a multi-agency framework



Provide therapeutic mental health assessment and intervention within a variety of community settings so to better engage young people



Utilise Routine Outcome Measures so to ensure partnership working which is goal specific



Collaborate and liaise with agencies, creating positive and helpful relationships



Support the eradication of stigma associated with mental health



Support partnership working through offering young people and families’ choice and flexibility in the service that they receive



Work in partnership with CAMHS clinicians, other senior clinicians and wider networks, to promote a robust Pathway.



Contribute to and support others in delivery of training, consultation and casework to other professionals and carer’s



Have a good understanding of national and local policies and procedures informing the evidence base for clinical care.



Promote the safety and welfare of service-users and their family



Co-ordinate and facilitate an appropriate discharge.



Maintain a professional attitude so to be a representative of the wider AMH service



To take a role in transitioning young people to Adult Mental Health Services, where appropriate provide therapeutic intervention.
