We are looking for dynamic, enthusiastic and committed specialist practitioner to support the delivery and the development of our Targeted Lung Health Check (TLHC) programme.


Southampton was one of only ten pilot centres in England to deliver targeted lung health screening to our local population. The programme is now expanding to new areas and this is a fantastic opportunity to support health promotion and reduce health inequalities. 


The team provide lung health checks to participants aged 55-74, who are or have been smokers and live in Southampton and surrounding areas and where required, will provide a low dose CT scan.

Detailed job description and main responsibilities

•    Work in the community environment to carry out the lung health screening programme and ensuring the smooth running of nurse-led clinics.
•    Provide support to nursing staff working on the mobile unit.
•    Provide participants with advice and support to promote healthy lifestyle, including smoking cessation advice and referring for ongoing support as appropriate. Communicate to participants in a way that empowers them to make informed choices about their health and health care.
•    Work in partnership with other healthcare professionals to carry out lung health screening and assess eligibility for low dose CT scans.
•    Ensure participants receive ongoing care that works in partnership with other healthcare professionals including referrals to alternative care pathways and treatments as clinically indicated.


You will need to be able to travel and work across multiple sites within Southampton. This role requires the ability to travel to and from hospital sites and other community settings.
