Job title: Service Coordinator

A fulfilling and exciting opportunity available for a Service Coordinator to join our Association. The Service Coordinator will be based in our offices in Denbighshire.

Purpose of the role

  • The primary purpose of this project is to provide support to people with multiple and complex needs as part of a Bury Bridges service.
  • The post holder is responsible for developing and delivering a person centred support package that is tailor made to suit the needs of each individual accessing the service. The support should assist service users in developing the skills required to build resilience to maintain independent living and either move on into permanent housing.
  • You will be expected to contribute to the maintenance and development of the Association’s values, culture and ethos.

  • Salary £23,985 per annum dependent on experience for 37.5 hours per week on a Fixed Term Contract until the end of March 2026,
