An exciting opportunity has arisen to work as a Community Respiratory / Oxygen Clinical Physiotherapy or Nurse Specialist in a Community Respiratory Service in Hounslow, as part of Integrated Care service line at West London NHS Trust.

This is an excellent opportunity for an enthusiastic and committed person to join new Community Respiratory Service in Hounslow. You will be working as part of a dynamic multi-disciplinary team comprising of nurses and physiotherapists, Therapist Assistant and administrative staff.

Hounslow Community Respiratory Service, supports chronic respiratory patients in the community with managing their conditions, providing them with support to Self-Manage. This job description outlines the role of a Community Respiratory Physiotherapist/Nurse responsible for managing respiratory patients and providing pulmonary rehabilitation, airway clearance, and breathlessness management clinics. It also highlights the developmental nature of the post, allowing a transition from Band 6 to Band 7, indicating that the post holder will be supported in developing skills, knowledge, and competencies to take on more advanced responsibilities over time.

To contribute to the care provision and review of the respiratory patient, thereby minimising risks and promoting recovery by supporting a multi-disciplinary approach to delivering individualised care, self-management and preventing unplanned hospital admissions.

To empower clients, adopting a problem solving approach, encouraging and giving clients the confidence and skills to improve the self-management of their chronic respiratory disease.

We are seeking a Respiratory / Oxygen Clinical Physiotherapy or nurse specialist to provide direct care for patients with Respiratory conditions in the community based Oxygen and Respiratory Case Management Team, including Pulmonary Rehabilitation and early hospital supported discharge for Hounslow residents using a multi-disciplinary team approach.

The Service Is To Provide

  • Pulmonary Rehabilitation service; Supporting patients to manage their breathlessness through exercises and improve their functional abilities
  • Home Oxygen Service – Assessment and Review (HOS-AR)
  • Case Management and support of patients with unstable or more complex respiratory disease.
  • Airway Clearance and breathlessness management clinic
  • Primary Care Education and Support.
  • Early Supported Discharge from hospital
