Mental Health Practitioner - Multiple Admission Patient Pathway Team
Sussex Partnership NHS Foundation Trust
Date: 2 days ago
City: Hastings
Contract type: Full time
An exciting opportunity has arisen for a Band 5 or 6 practitioner to join our Multiple Admissions Patient Pathway (MAP) Team based within our Assessment and Treatment Team in Hastings. The MAP team was created 18 months ago as a pilot project to work with individuals who present with complex mental health needs, are high users of services and at risk of multiple admissions to hospital due to frequent cycles of crisis and difficulties engaging in community support.
This post offers the Trust's £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).
The team is a multidisciplinary team who work using dialogical and systemic principles based on the open dialogue approach. They offer an enhanced level of support that is delivered via regular network meetings alongside other services to improve trust and engagement in the community offer of support. They work closely with carers/families and other agencies to ensure a joined up and consistent approach to care.
The project has worked with 11 patients in the last 18 months and there has been significant positive outcomes for all patients. This includes a reduction in bed days, reduction in crisis presentations/episodes and reduction in A & E presentations. Some patients have also been discharged from the pathway. We are now looking to recruit substantively to the in Hastings whilst we look to expand across East Sussex into other ATS's.
Sussex Partnership is a fantastic Trust to work for and nursing is highly valued. Our teams are welcoming and enthusiastic about providing the best possible care for people who access our services. Sussex is a beautiful county with good transport links, vibrant urban communities, rural areas and tourist attractions, including the Coast and the South Downs. The Trust has recently been rated by the CQC as Good overall and Outstanding for care.
