Job Introduction

As a Rapid Response Sister / Nurse you’ll be working as part of the Intermediate Care team in Surrey Heath area based in Camberley and also be part of the In-Reach Team that works Farnham Community Hospital.

You will actively working alongside Therapists and Community rehab assistants as well as the wider MDT in Surrey Heath to, proactively support patients on the caseload to remain independent within their own home environment, providing care planning, preventing unnecessary hospital admission, support flow/transition through the service..

We are a 7 day service, weekend and bank holidays included within the shift patterns.

Bank holidays are split between the team, with one Saturday OR Sunday each month.

Working hours are between 8am to 4pm we are flexible with the hours worked with only one Duty Shift (12pm-8pm) per week.

Main Responsibilities

As aIntermediate Care Sister, you will work alongside the therapist's and other members of the Community MDT to support patients to live safely and independent in their own homes. You will assess patients in their own home for Admission Avoidance , as well as supporting and assessing them following a fall at home.

In the Hospitals you will work and liaise with the therapy teams to support and assess patients who are being discharged home or on the wards.

The Ideal Candidate

You will have a nursing qualification and be on the NMC register.

Ideally you will have community experience but this is not essential, Ward, Rapid Response and Care Home experience is also considered. Having the Health Assessment qualification is preferred.

You will have excellent communication skills and be able to work in a busy environment, which requires flexibility at times.

Must be confident using IT systems and EMIS experience desirable but not essential.
