In this unique position, you’ll deliver high-quality, person-centred care through a combination of clinical nursing and social work approaches. From discharge planning and safeguarding to care coordination and family liaison, this role plays a vital part in supporting service users through recovery. You’ll work closely with a dedicated multidisciplinary team, external agencies, and families to ensure safe, effective care that respects each individual’s needs and rights.

You will be working at the Gateway Recovery Centre, a state-of-the-art facility which provides care for men and women with complex mental health requirements.

As the Integrated Nursing and Social Work Practitioner you will:
- Provide specialist input across nursing and social work areas such as safeguarding, discharge planning, housing and care coordination.
- Support holistic assessments and contributing to person-centred care and risk management plans.
- Liaise with families and external agencies to support care continuity and statutory responsibilities.
- Promote ethical and accountable practice in line with NMC and Social Work professional standards.
- Contribute to safeguarding adults and children, including taking a lead role where appropriate.
- Participate in multidisciplinary team discussions to influence care planning from both clinical and social perspectives.
- Support quality assurance, governance processes and compliance with relevant legislation and policy.
- Deliver and receiving supervision, engaging in professional development, and supporting team learning.
- Appropriate dual qualification in RMN and Social Work
- Evidence of other development and/or training which indicates a commitment to continuous development and preparation for management.

The Hospital Is Split Into Four Distinct Areas
- Male Autism Service for the challenging and complex needs of adults with Autism and Asperger’s syndrome
- Assessment and rehabilitation for women with complex mental health needs and personality difficulties
- Specialist inpatient service for older men with complex mental health and physical health needs.
- Low secure care setting for men with a mental illness with or without personality disorder/mild LD
