Area East Continuing Healthcare Care Team have an unique opportunity for an enthusiastic, motivated RMN for the role of Continuing Health Care Lead Nurse OPMH Band 7.
The role involves managing a team of Nurse Reviewers who visit patients in care homes and patients own homes to review and assess their needs in relation to NHS Funding, in accordance with National guidance. The post holder would work closely with the Discharge teams across hospital sites, Nursing Homes, Social Services departments, and other professionals within the NHS, including contract teams. The post holder will co-ordinate, scrutinise CHC applications and participate in CHC and FNC reviews and attend and participate in Multi Disciplinary Team (MDT) meetings within the area/department where required. The post holder will be responsible for providing training packages for Multi-Agency staff on CHC issues and promoting good practice.
Working within Continuing Health Care is a challenging role and involves dealing with sensitive information and skilled communication is a vital part of the role. There is an expectation that the candidates will have knowledge of the Continuing Health Care Framework and understanding and experience of assessing patients for continuing health care.

Main duties of the job
Implementation and monitoring of Continuing NHS Health Care funding (CHC) in Care Homes and Domiciliary care packages in accordance with the national Framework for CHC and NHS Funded Registered Nursing Care (FNC).
Extensive joint working with the Local Authorities ensuring patients in the Independent Sector funded by BCUHB receive a high quality and safe service.
Co-ordinate, scrutinise CHC applications and participate in CHC and FNC reviews, attend and participate in Multi Disciplinary Team (MDT) meetings within the Area where required.
Manage the local resolution process within the Area i.e. meet and liaise with families to discuss panel outcomes when individuals challenge eligibility decisions.
Responsible for the local resolution process within the Area for Inter Agency Disputes – i.e meeting and liaising with the appropriate Team Manager from the Local Authority where there is a dispute regarding eligibility for CHC funding.
The post holder will work in collaboration with the statutory agencies where safeguarding issues have been identified and will attend POVA,/Strategy meetings.
Working collaboratively with CMHTs, DN Services; Discharge liaison Teams to ensure patients are discharged / transferred in a safe and timely manner.
The role will support the management duties of the CHC team locally.
The post holder will be responsible for providing training packages for Multi-Agency staff on CHC.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
