The team work within Derriford Hospital to support vulnerable and complex customers who are either homeless, at risk of homelessness or require support to maintain their tenancies. The team work closely with other hospital departments to facilitate safe and appropriate discharges.
Position:
In this role you will provide support and case management to people with multiple and complex needs using The Plymouth Alliance (TPA) principles. It would be great if you have previous experience of working with customers experiencing homelessness and or other socially excluded people and a good understanding of safeguarding issues. You will also have the energy and enthusiasm to provide positive support and trauma informed care to those who need our support. It is a role where you can really make a difference.
You will be working as part of a team to provide support to our customers. You will love helping others and enjoy supporting individuals to achieve their goals and to become more independent. Your positive attitude towards your work and others will help to contribute to a great working environment. You will have a calm and personable manner and be able to handle potentially difficult situations.

Able to demonstrate you are an effective team worker (and able to work effectively on your own, if required)
Able to make sound decisions
Driver with access to own vehicle
