Job Overview
We are seeking to appoint an enthusiastic Community Administrator to support our clinicians and service users, within the Long Term Conditions Team who are based at Charnwood Mill in Barrow upon Soar.
Our service operates 08:00-17:00 weekdays and shifts will be allocated to ensure the service is covered. We are looking for someone to join us three days a week, including Thursday and Friday, but there may be some flexibility with this in the future.

Job Description Summary (Main Duties And Responsibilities)
Receiving and dealing with calls, queries, referrals and tasks from professionals and handling these as required, in order to coordinate patient care according to agreed pathways.
To work under the direction of the Admin Team Leads as part of an effective team to provide comprehensive administrative support to achieve key objectives of the Long Term Conditions team.
To be responsible for effective communication with patients and carers as well as a wide range of health and social care professionals to ensure the service runs efficiently and effectively
