An exciting opportunity has arisen in the Forensic Community Learning Disabilities Team (FCLDT) for an experienced specialist practitioner to join the team for a fixed 12-month period.
We are a small specialist community based service that is strongly committed to multi-agency working. We work closely with local learning disabilities services, social care, mental health services and the criminal justice system. We provide forensic risk assessment and consultation on risk and the management of offenders who have Learning Disabilities, as well as individualised offender treatments.
Based in Petherton Road resource centre in South Bristol .We provide a service across BNSSG (Bristol, Bath & North-East Somerset; South Glos and North Somerset) providing direct assessment ,intervention and consultation to people with learning disabilities who may be at risk of offending or who may have offended. The team provide a consultancy based service across Swindon and Wiltshire working collaboratively with members of the Multi-disciplinary and Multi agency teams.
We are seeking an enthusiastic and experienced practitioner to join us in our friendly and supportive team. The ideal candidate will have experience of working individuals with learning disabilities who may be at risk of coming into contact with criminal justice services or who have a history of offending behaviour. The post requires an emotionally resilient person who is also a strong team player.
Work with local Learning Disabilities service to help assess referrals for eligibility for the Forensic Community LD Team
Raise awareness, in all areas of health, social services, prisons, probation
services and courts, of the needs of people with LD who are at risk of offending.
This involves formal and informal teaching.
Assist the team with forensic risk assessments
Provide individual interventions with service users aimed at helping them manage and reduce their offending risk
To hold an allocated group of service users, formulating, implementing and evaluating care plans and treatment programmes accordingly
Promote fair access to services for people with LD who are at risk of offending
Provide information to service users in a manner that is accessible
Provide specialist knowledge and opinion to the service user, carers and others working with the service user regarding the service users needs, treatment and risk management
Ensure good links are developed and maintained with community and inpatient LD services, facilitating referrals where appropriate
Organise review meetings and chair when appropriate
To participate in and chair when required, Team Business Meetings and Allocation meetings.
To be responsible for the protection of individuals from abuse and harm in line with local safeguarding policies and procedures, including working with others in the planning development and review of protection plans, ensuring the appropriate sharing of information.
