Contract type: Full time
Are you Dynamic? Motivated? Passionate about community based care? A registered Mental Health Nurse?

If you can answer yes to these questions, then we want you to apply for our band 6 post in the Dementia Service. We are redesigning the model of care based on identified pathways and looking for clinicians who are skilled or have an interest in training to deliver interventions to service users living with Dementia.

This is an exciting opportunity for clinicians to work in a focused way developing skills for an identified group of individuals living with Dementia and improving the outcomes and experience for our service users and their carers.

The successful applicant will be responsible for clinical assessments of service users referred into the service through a 'duty system'. Assessments may be conducted initially over the telephone to gather the required information to formulate a plan of care. Dependent on need, there may be a requirement for face to face assessment. The role is primarily focused on establishing an effective 'duty system' but may require care-coordination of service users open to the caseload, dependent on the needs of the service.

The role will involve providing therapeutic interventions and also linking in with other professionals to co-ordinate their interventions, this will all be done within the CPA process.

Main Responsibilities

Initial assessment with service users living with dementia who are presenting with behavioural and psychological symptoms of dementia. Due to the nature of the client group, initial assessment is likely to include working with family and carers.

Multi-agency working.

Risk management working.
