Primary Care Associate Practitioner (Social Prescribing)
NHS Tayside: Band 4, £29,116 - £31,670 per annum
DCC Grade 7: £27, 511 £32,084.02 per annum
Fixed term to 31/03/2026

Sources of Support works alongside patients in Dundee GP Practices whose physical and mental health and wellbeing is impacted by social and economic issues and personal circumstances. The service offers non-medical interventions and co-ordinated care to help improve patients’ quality of life, whilst alleviating the pressures placed on practices due to non medical issues.
An exciting opportunity has arisen within the service and we are looking for a highly skilled practitioner to join our team as a Primary Care Associate Practitioner (Social Prescribing). Our team consists of 8 Primary Care Link Workers and 2 Primary Care Associate Practitioners (support workers). The post holder will work closely with the Primary Care Link Workers and work with patients face to face in health centres and community settings.
The successful candidate will have a caseload of up to 35 patients with complex needs and work with them for up to 12 weeks to help them achieve their identified goals. The Primary Care Associate Practitioner will work alongside the Primary Care Link Workers, playing a key role in supporting patients to access a range of activities which support health improvement. You will contribute to the delivery of community-based interventions to improve life circumstances, support lifestyle changes and tackle the determinants of health and wellbeing using a motivational interviewing approach.
This is a fast-paced role and candidates will have excellent communication skills and must be able to demonstrate experience of working with people on a one to one basis who have complex social, economic and personal circumstances. Candidates should have experience of multidisciplinary and collaborative working with partners within a range of work settings.
Applicants must be educated to SVQ 3 in a relevant area.
Internal - Written approval must be sought from your line manager prior to applying for this secondment. You will be required to bring this if shortlisted for interview.
You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct throughout your employment.
The service has recently moved into DHSCP Primary Care structure and sits within The Health Inclusion Service. Please note the job descriptions and job specs do not reflect this move.
