Job Overview

We have exciting opportunities for several Band 4 Assistant Practitioners to join the Community Childrens Nursing Service. The role will support the Children and Young People with high complex physical needs following safe delegation and with guidance from a Registered Nurse in Continuing Care in Children and Young People homes and other establishments.

Main duties of the job

The Assistant Practitioner will work under the direction of the registered nurse to deliver high quality, holistic, compassionate health care to children and young people with high complex needs, providing the required support for families.

The post holder is expected to independently manage their own work and case load, undertaking tasks delegated by a registered practitioner with appropriate supervision in place. Having an understanding of evidence based practice and delivering care in line with current evidence, the post holder will take responsibility for taking action relative to an individual’s health and care in accordance with organisational policy and procedures

The post holder will be responsible for some elements of assessment, implementing programmes of care and modifying individualised care plans, reporting back to the registered practitioner. They may delegate work to others and may supervise, teach and assess other staff.

Qualifications And Knowledge

Essential criteria

  • Certificate in Health Care Level 4
  • Agored Principles of Administration of Medication
  • Agored - Fundamental skills for administration of medication

Desirable criteria

  • Evidence attendance at courses / study days pertinent to speciality
  • Successfully undertaken Agored Cymru accredited Unit on Administration of Medication

Experience

Essential criteria

  • Administration of medication via oral, topical, NG, Peg, Jeg routes
  • Interpersonal and communication skills and the ability to manage/ diffuse challenging or threatening situations.
  • Recent clinical experience of working within a relevant healthcare service environment

Desirable criteria

  • Experience of working as a healthcare assistant in a community setting
  • Experience of supporting the development of others

Aptitude and abilites

Essential criteria

  • Ability to function as an Assistant Practitioner/ without direct supervision
  • Able to undertake patient assessments within agreed competency and protocols
  • Ability to develop effective and appropriate relationships with patients, their families, carers and colleagues
  • Ability to support, supervise, assess, and act as a role model for Trainee Assistant Practitioners other learners and HCAs as required within the clinical setting

Desirable criteria

  • Ability to speak Welsh
  • ANTT (Aseptic Non-Touch Technique)
  • Ability to effect change in a positive manner

Other

Essential criteria

  • Able to participate with a rotating shift pattern.
  • Excellent timekeeping and a flexible approach to work
  • Able to meet the physical demands of the job such as moving and handling patients and assembling, operating and maintaining clinical equipment
