JOB PROFILE

The post holder will care for patients in the community and work within the Ambulatory Outreach Team (AOT). Assist in the development, implementation, delivery and evaluation of the AOT [Acute Hospital at Home (AHaH) & Complex Outpatient Parenteral Antimicrobial

Therapy (COPAT)] function and contribute in further development and streamlining of the AOT. As a senior nurse, the post holder is expected to work and provide specialist input, and flexibly rotate within both services.

The post holder is expected to work autonomously and support other team members. Care for patients in their home environment, support and improve the patient discharge process for patients who can safely be managed at home with a defined care plan. The post holder

needs to be able to perform holistic clinical assessment for Admission Avoidance patients (training will be provided) at the patient’s home.

As such our Ambulatory Outreach Team and Hospital at Home service is expanding as we take on more work including covid work streams. Existing band 6’s ready for a new challenge and the opportunity of learning new clinical skills. Due to the nature of our work a min of 2 years practice in an acute area of work is a must.

The Ambulatory Outreach Team (AOT) comprises of numerous roles which include Hospital at Home Central, Hospital at Home Horton, OPAT (Outpatient Parenteral antimicrobial Therapy) and Covid Care at Home. We also work closely with the Ambulatory Assessment Unit (AAU) at the JR and Rowan Ambulatory Unit (RAU) at the Horton. Therefore, there is a variety of work including hospital-based work, community work and the opportunity to work from home with covid care at Home service.
