We are seeking an organised and empathetic Admission & Discharge Co-Ordinator to ensure seamless transitions for our patients, providing crucial support in managing the admission and discharge process within our facility.
Position Overview
The Admission & Discharge Co-Ordinator plays a vital role in coordinating patient transitions, working closely with clinical teams and families to facilitate smooth admissions and discharges. This position requires meticulous attention to detail and a strong commitment to patient care, ensuring that all processes are optimally managed and documented.
Key Responsibilities
Contract: Permanent
Hours: Full Time, 37.5 hours per week
Location: Basildon Hospice and Thurrock Hospice

Key Responsibilities
- Coordinate with medical staff to schedule and manage patient admissions and discharges.
- Maintain accurate records of patient data and ensure compliance with medical regulations.
- Communicate effectively with patients and families to explain procedures and provide support.
- Collaborate with multidisciplinary teams to optimise patient care plans and transitions.
- Identify and address any issues or delays in the admission and discharge process.
- Ensure all legal and ethical standards are met during the transfer process.
- Previous experience in a healthcare administrative role or similar capacity.
- Exceptional organisational and multitasking skills.
- Strong communication skills to liaise with healthcare professionals and families effectively.
- Proficiency in using healthcare management systems and software.
- Empathy and understanding of the challenges faced by patients and their families.
Contract: Permanent
Hours: Full Time, 37.5 hours per week
Location: Basildon Hospice and Thurrock Hospice
Additional Requirements
- Ability to work flexible hours, including evenings and weekends, as needed.
- Right to work in the UK; sponsorship is not available.
