Purpose of the role:
Are you a strategic thinker with a passion for integrated health and wellbeing services? Are you passionate about improving public health and supporting communities to lead healthier lives? We’re looking for a dynamic System Integration Lead to drive innovation, collaboration, and service excellence across our One You Kent Service, covering the entire county.
The System Integration Lead provides strategic, system-level leadership to embed One You Kent lifestyle services Smoking Cessation, Weight Management and Lifestyle into local care pathways across Kent.
The postholder will work across Integrated Care Boards (ICBs), Primary Care Networks (PCNs), Local Authorities, VCSE partners and NHS providers to design, implement and sustain integrated referral pathways, ensuring prevention and lifestyle interventions are embedded as a routine part of care.
This role is outward-facing and influence-led, focused on system engagement, pathway redesign, referral integration, and service growth in terms of scalability and income generation, enabling One You Kent to reach priority populations and maximise impact at scale.
Our services deliver high-impact health interventions tailored to local needs, providing holistic support through Smoking Cessation, Weight Management and Lifestyle Advice programmes.

Additionally, existing experienced delivery teams will be available to support you to drive the Service forward.
Key Duties and Responsibilities:
The successful candidate will play a key role in building and strengthening strategic relationships with Integrated Care Boards (ICBs), Primary Care Networks (PCNs), local authorities, NHS providers, and VCSE partners to support the integration and development of lifestyle services across Kent. The role involves identifying opportunities to integrate services into existing care pathways, co-designing new and inclusive referral models with system partners, and leading pilot projects and test-and-learn initiatives to inform wider service development and scale-up. You will work closely with clinical and digital teams to embed referral mechanisms within primary care and community systems such as EMIS and SystmOne, ensuring pathways remain accessible, responsive, and easy to use, while supporting referrers through engagement, communication, and training. In addition, you will identify opportunities for service growth, innovation, and investment, contributing to the development of business cases, bids, and proposals aligned to population health priorities, and supporting partnership models that enhance sustainability, reach, and social value. The role also includes defining success measures and KPIs for new initiatives, using data and feedback to evaluate impact and drive continuous improvement, and providing clear reports and updates to internal leaders and system partners. You will support frontline teams and partner organisations to maximise the use of referral pathways, share learning and best practice across the system, and provide leadership and oversight to Stakeholder and Engagement Officers.
