- Individual Outcome and Wellbeing: Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
- Activity Planning: Develop flexible, engaging activity plans that promote autonomy and personal growth.
- Health and Lifestyle: Support individuals in maintaining healthy lifestyles and managing their health needs.
- Communication: Facilitate effective communication and maintain detailed, person-centered records.
- Medication Management: Oversee medication processes and support individuals in self-administration.
- Safety and Environment: Maintain a safe, clean-living environment and adhere to health and safety protocols.
- Compassionate and empathetic
- Strong communication skills

- Committed to promoting diversity and inclusion
- Team player with a collaborative spirit
- Experience Preferred, have or working towards at least Level 3 in Health and Social Care.
- Must hold a full UK driving license; driving will be undertaken using a service user’s / employer-provided vehicle.
