West Berkshire Council's Adult Social Care Service is seeking dedicated Social Care Practitioners to join our team and contribute to the delivery of high-quality social care services.

The Role

As a Social Care Practitioner, you will be responsible for:

• Working collaboratively with individuals to assess their social care needs and provide appropriate support and interventions.
• Conducting comprehensive assessments and reviews of individuals' care requirements, ensuring their needs are met.
• Building positive relationships with service users, their families, and other professionals to promote effective care planning and delivery.
• Providing advice, guidance, and advocacy to individuals, empowering them to make informed choices about their care.
• Ensuring compliance with relevant legislation, regulations, and organisational policies.
  
  

This is a fixed term contract 1 year from start date (Funding limited project)

The Ideal Candidate

The ideal candidate will possess the following skills and attributes:

• A relevant social care qualification and registration with the appropriate professional body.
• Experience working in a social care setting, ideally within an local authority environment.
• Sound knowledge of social care legislation, regulations, and best practices.
• Excellent assessment and care planning skills, with the ability to identify and address individuals' needs.
• Strong communication and interpersonal skills, with the ability to build rapport with service users and collaborate with multidisciplinary teams.
• Ability to work independently, manage a diverse caseload, and prioritise tasks effectively.
• A positive and solution-focused approach, demonstrating resilience in challenging situations.
