You will build meaningful relationships with patients and play a key role in supporting their health and wellbeing in a more personal, one to one setting. In this community based role, you will support the delivery of nursing care to patients in their own homes and local clinics. This is a varied and active position where no two days are the same. You will play a vital role in helping patients manage their health, recover from illness and maintain independence. You will work closely with registered nurses and the wider multidisciplinary team, carrying out delegated care and developing your clinical skills. You will also have the opportunity to work independently, planning your visits and managing your time effectively across the local area. Key responsibilities include:

- Carrying out clinical tasks such as dressings and glucose monitoring following training and competency sign off
- Supporting holistic, evidence based care that meets each patient’s individual needs
- Maintaining accurate patient records using electronic systems
- Assisting with administrative tasks to support the team
- Planning and organizing your own daily visits across the community
Person Specification
Essential
- NVQ Level 2 or 3 in Health and Social Care, or equivalent experience
- A genuine interest in community based care and supporting patients in their own homes
- Full UK driving license and access to a car
- Good communication and interpersonal skills
- Ability to work independently and manage your own workload
- Basic IT skills including Microsoft Office
- Previous experience working in a community or domiciliary care setting
- Experience supporting a wide range of patient needs in a non hospital environment
- Confidence working autonomously in the community
