Diabetes Specialist Nurse (Band 7)

A secondment opportunity has arisen for an experienced Diabetes Specialist Nurse to join the Diabetes Team at Wexham Park Hospital on a full-time basis.

The successful candidate will provide specialist diabetes care for adult inpatients and support ongoing follow-up within outpatient and community clinics. Working as part of a multidisciplinary team, the post holder will coordinate treatment plans, deliver evidence-based care, and provide education and support to patients and their families to promote effective self-management of diabetes.

The role includes supporting complex diabetes management across the hospital, ensuring continuity of care from admission through to discharge and community follow-on services. The post holder will also contribute to maintaining and improving professional standards of diabetes care in line with local and national guidance.

As a specialist practitioner, the Diabetes Specialist Nurse will act as a key resource for medical, nursing, and allied health professionals, providing expert advice, education, and clinical support across the Trust. The role offers an excellent opportunity to contribute to service development and enhance patient outcomes within a busy acute hospital setting.



Main duties of the job

To work as part of the multidisciplinary team providing a high-quality specialist diabetes nursing service for adults with diabetes across inpatient, outpatient, and community settings. The post holder will deliver expert clinical care, education, and support to patients and their families, promoting effective self-management and continuity of care following hospital discharge.

The role involves assessing, planning, implementing, and evaluating individualised care and treatment plans for patients with diabetes, including those with complex needs. The successful candidate will support diabetes management within the hospital environment and provide follow-up care within outpatient and community clinics.

The post holder will be responsible for maintaining and enhancing professional standards of diabetes care in line with local and national guidelines, contributing to service development, clinical audit, and quality improvement initiatives.

Acting as a specialist diabetes resource, the Diabetes Specialist Nurse will provide expert advice, guidance, and education to medical staff, nursing teams, and allied health professionals across the Trust, supporting best practice and improving patient outcomes.

Qualifications

Essential criteria

  • Current NMC registration
  • To have a recognised diabetes course
  • Nurse prescribing course (NMP)

Desirable criteria

  • Possess a relevant Nursing Degree or be willing to work towards one

Experience

Essential criteria

  • Previous post registration NHS experience in a diabetes environment
  • Teaching and supervising of junior staff and students
  • Working with IT systems e.g., patient centre

Desirable criteria

  • Experience of audit/research Publication

Skills & Knowledge

Essential criteria

  • Excellent interpersonal skills with the ability to manage difficult/stressful situations effectively
  • The ability to work independently to an approved protocol whilst acknowledging limitations and liaising with to Senior DSN or Diabetologist where appropriate.
  • Supervising and teaching fellow colleagues and students
